Individualized Family Support Plan (IFSP) Document

The Individualized Family Support Plan represents a family-centered planning process that results in a written plan describing the outcomes the family would like to see achieved, the services and supports for the child and family, and the coordination of these services and supports. The Individualized Family Support Plan process recognizes the family as a whole and understands the important role the family plays in making early intervention successful.

Early Steps uses a standardized document in all 16 of the Local Early Steps. The IFSP document is a fluid document that reflects the Early Steps family-centered philosophy and the empowering nature of the IFSP process. The Early Steps IFSP document contains the eight required components of the IDEA, Part C, as described in the previous section.
